Summary of Society for Maternal-Fetal Medicine (SMFM) Consult Series #52: Diagnosis and management of fetal growth restriction - Contemporary Obgyn

- March 13, 2021

Created in partnership with the Society for Maternal-Fetal Medicine. SMFM was established in 1977 to "give Maternal-Fetal Medicine (MFM) physicians and scientists a place to share knowledge, research and clinical best practices in order to improve care for moms and babies." Introduction Fetal growth restriction (FGR) can result from a variety of maternal, fetal, and placental conditions. It occurs in up to 10% of pregnancies and is second to premature birth as the most common cause of infant morbidity and mortality. 1-3 Terminology & diagnostic criteria FGR is commonly defined as an ultrasonographic estimated fetal weight (EFW) below the 10th percentile for gestational age. Small for gestational age (SGA) describes a newborn whose birth weight is less than the 10th percentile for gestational age. A challenge in the prenatal management of FGR is differentiating the constitutionally small fetus with a normal neonatal outcome from one who is pathologically growth...

BTS Clinical Statement on air travel for passengers with respiratory disease - Thorax (BMJ)

- March 27, 2022

Introduction BTS recommendations for managing passengers with stable respiratory disease planning air travel were published in Thorax in 2011.1 This followed original guidance published in 20022 and an online update in 2004.3 The 2011 recommendations provided an expert consensus view based on literature reviews, aimed at providing practical advice for lung specialists in secondary care. Recognising that knowledge in this area has grown since 2011, and that updated, pragmatic advice regarding which respiratory patients need specialist assessment is required, the Society has commissioned a new clinical statement. Although air travel appears generally safe for those with respiratory disease assessed previously by a lung specialist,4 a decision to undertake air travel should not be taken lightly. Diverted flights incur significant expense and inconvenience, and a patient whose condition deteriorates during flight can pose huge challenges to airline crew and other passengers. High altitude...

A pediatrician explains a spike in ear infections this summer after COVID-19 restrictions lifted - The Conversation US

- September 30, 2021

Ear pain is one of the most common reasons that young children go to the doctor, and acute otitis media – which means "middle ear infection" – is the most frequent cause for the use of antibiotics in children under 5 years of age. Twenty-five percent of children will have an ear infection by their first birthday, and 60% by age 5. As a pediatrician, I see children in my clinic daily for ear infections. Because these are associated with viral upper respiratory infections, we typically see most ear infections in the fall and winter, when influenza and cold viruses are prevalent. However, with near-universal mask-wearing due to COVID-19 last winter and many children out of school, the number of viral upper respiratory infections dropped drastically. As a result, we saw very few ear infections in our clinic. This summer, with the lifting of COVID-19 restrictions, we have seen a return of many respiratory viruses, and with them, middle ear infections. How an ear gets i...
